Brown meat and onions. Drain. Add tomato puree, 2 tsp. salt, 1 tsp. cinnamon, 1 tsp. pepper. Mix well and cook for 20 minutes. Set meat mixture aside. Meat may be prepared in advance or day before.
Add macaroni to boiling salted water.Boil 10 minutes. Drain and rinse macaroni. Transfer macaroni to a large aluminum pan for mixing. Add one stick of melted butter to the macaroni and mix well. In a mixer, beat three whole eggs and three egg whites until fluffy. Pour the egg mixture over the macaroni and mix well. Add 1 ½ cup Romano cheese to the macaroni, butter and eggs and mix well. Butter the bottom and sides of the pans. Place half the macaroni mixture in the bottom of each pan. Divide the meat mixture and place evenly on top of the macaroni.
Step Three: (Crema)
You'll need a pot large enough to hold ½ gallon of milk and a large whisk. Warm ½ gallon whole milk in the microwave or in another pot, (not the pot you plan to make the crema in.) For crema, melt 2.5 sticks of butter, careful not to burn. Add 1 ½ cup flour ½ tsp. salt, ¼ tsp. pepper and stir constantly. Add ½ gallon warm milk + 3 egg yolks and continue stirring until the sauce thickens. Once the sauce is thick, pour evenly over the two pans of meat & macaroni mixture. If planning to freeze pasticho, do not cover until the pan has cooled completely. If planning to cook, place uncovered in a 350 oven (on a cookie sheet) for one hour. Let pasticho cool or 20 minutes before cutting and serving.
